Making Your Wishes Known for End-of-Life Care
We are — each of us — entitled to a good death with pain and symptoms under control. Death should not devastate individuals and families financially or emotionally.
Caring Conversations® is a consumer education initiative that helps individuals and their families share meaningful conversation while making practical preparations for end-of-life decisions. The goals are as follows:
- Shift the focus of advance care planning to include conversations with family and friends
- Help families advocate on behalf of those who can no longer speak for themselves
- Ensure that the wishes of those near the end of life are honored
- Educate healthcare providers who participate in advance care planning
Each copy of Caring Conversations includes a healthcare directive document and a durable power of attorney for healthcare decisions.
