Living wills are ineffective, primarily because they are too ambiguous to guide medical decision making and because the problem they were designed to address no longer exists to any significant degree. Attempts to reformulate instructive directions and make them more clinically relevant are unlikely to succeed. Responsibility for avoiding inappropriate care at the end of life lies not with patients, but with health care practitioners.
