Health is related to conditions in our lives, known as social determinants of health. These conditions exist long before any illness – in our homes, schools, jobs and neighborhoods. The need to address social determinants of health is especially urgent for health systems serving minority and low-income populations and for nonprofit hospitals.

In this webinar, we invite you to participate in a conversation with Dr. Sanne Magnan about what you can do to address the social determinants of health and to advance health equity in your communities.

Dr. Magnan is the previous co-chair of the Roundtable on Population Health Improvement of the National Academies of Sciences, Engineering and Medicine (NASEM). Currently, she is co-chair of the Health Care Expenditure Collaborative associated with the Roundtable.

Talia, her physician father and her mother pleaded with doctors and medical staff, urging them to recognize that Talia couldn’t breathe. Talia’s parents have a powerful story to share.
